1) Describe your population of interest.

The African American population is one of the most diverse populations in the US, with the group comprising people of varied ethnic-racial experiences and identities. In the US, the African-American population includes those who identify themselves as black along with other racial backgrounds or alone. By 2019 the US Census Bureau estimated that there were at least 46.8 million African Americans across the US who made up at least 14% of the country’s population. In the US, the African American population is made up of alteration and Hispanic black people and single-race non-Hispanic black people. All individuals who identify as African Americans, therefore, trace their origin to the African continent (Ahrq.gov, 2018).

2) What are the top five health conditions affecting this population?

The top five health conditions affecting African Americans include heart diseases, stroke, asthma, cancer, diabetes, influenza, pneumonia, and HIV/AIDS. African Americans will therefore be more susceptible to the highlighted conditions compared to their White counterparts (Noonan et al., 2016).

3) Which one of the top five common health conditions in your population of interest have you chosen for the Population Health Poster Assignment?

For the population health poster assignment, I chose diabetes as one of the most common health conditions that affect the African American population.

4) Why did you choose this health condition?

I chose diabetes because, according to the CDC, the African American population is disproportionately affected by the condition, with African Americans being more than 60% more likely to suffer from diabetes compared to White Americans (Spears et al., 2017). African Americans are also more susceptible to negative health effects as a result of diabetes, such as end-stage renal disease, and other negative outcomes such as lower-extremity amputations compared to other populations. African Americans are also more likely to struggle to control their diabetes compared to patients from other races. It is important for healthcare professionals, including advanced practice nurses, to come up with solutions that can help to control the prevalence of diabetes in the African American population (Spears et al., 2017).
