Questions such as how the care plan will be executed and the positive changes which the nursing care plan hopes to achieve in a patient’s life should be addressed. It is also important to discuss details of who will be responsible for the delivery of the care plan and the measurable goals that will be tracked to determine the success of the plan. As you complete the paper, you should discuss your personal recommendations. The recommendations should be based on the current status of the patient, the diagnosis, prognosis and nursing care plan.
