Evidence-based practice in nursing means being guided by data from various sources of data in gaining quality information that can determine an action to be taken for the effective functionality of the health system. Many EBP models guide the application of evidence-based practice in health processes. The Johns Hopkins Nursing EBP Model is one of the commonly used models with three phases of application: the question, collecting evidence, and then translation (Ryan et al., 2019). After collecting the findings, the health team or researcher analyses them in the translation phase puts them into practice in the healthcare system. What follows is the evaluation process of the change.
Health promotion and disease prevention are essential across all cultures and ages. Health promotion and disease prevention activities such as health education, disease screening, health monitoring, physical activity, substance use cessation programs help reduce healthcare gaps and inequalities among the people of different cultures within a community. Culturally centered health promotional activities help reduce bias in healthcare, promote trust and the quality of care, leading to higher rates of diagnosis and patient adherence to health improvement activities.
The health assessment process provides a quality interaction with a patient that would enhance the quality of diagnosis and treatment processes. The assessment across lifespan involves people of different ages from infant to elderly, focusing on the key health factors affecting a community section. The assessment across lifespan involves focusing on social factors that include the social environment in establishing the healthiness of the surroundings. For infants, the healthcare provider will establish issues such as availability of shelter, quality food, medicine, and general caregiving. The youth’s social life influences health and can be assessed through social cohesion, levels of education, and behaviors (Helga, 2018). The adults can be socially assessed by providing a quality social network, social amenities, and basic needs such as food, healthcare, and a quality environment. The second factor to consider is the cultural beliefs that affect general health, including infants, youth, adults, and the elderly. Nurses will establish the beliefs that may contribute to health adversities in positively challenging them for better health outcomes. Other factors include the political and economic factors that will help the healthcare provider determine the quality of the general environment for healthiness.
Implementing an EHR system in a healthcare facility helps in digitizing and storing patient records electronically and enhancing communication among healthcare providers and their patients. The change project involves changing from paper-based cabinets to an EHR system. In applying evidence-based practice, there should be an effective plan. The process starts by designing the questions on the effectiveness and cost-effectiveness of the EHR system. The second step is to search for evidence in the virtually available databases. Some of the most revered databases for nursing practice include DOAJ and PubMed. The databases are scientific, providing peer-reviewed sources for medical practices. After acquiring data from the sources, the third step will involve analyzing the data from the sources to be information applicable in nursing. The fourth step is applying the information acquired on whether the EHR system will improve the quality of the healthcare process. The other step is on evaluation of the implemented project.
