This module forms part of the Master of Science in General Practice. This module follows on from the General Practice and Health Systems module by practical placement in a variety of relevant self-selected community based services. The aim of this module is to increase awareness and knowledge of services in the community that improve patients’ health and well being by addressing physical, psychological or social issues that can negatively impact on health.

Learning Outcomes:

At the end of this module students will be able to: 1. Demonstrate increased awareness of community based services 2. Reflect on services from the perspective of the patient, the service, the community and general practice 3. Discuss how the service may impact on patient health and well being 4. Describe and analyse key issues in relation to the interface between community based services, general practice and secondary care services.
