Theoretical Foundations
Nursing theories are guidelines defining the nursing profession and its operations. Theories define the uniqueness of nursing while acting as a framework for guiding nursing practice. These theories act as the basis for positively influencing health and quality of patient life (Hoeck & Delmar, 2018). Nursing theories have also been used as guiding tools for research and education. In research, nursing theories are fundamental in guiding the research study. Theory creates the phenomenon of interest in the study so that the study meets its goals. This paper explores the use of grand and middle-range nursing theories in research and explains the significance of theory in advanced nursing practice.
Background on Theories
Grand Nursing theories have a broad scope in the professional addressing a wide range of concepts and propositions encountered in the daily nursing practice. These theories are mainly oriented towards conceptual frameworks and models defining the profession in different situations, care environments, and phenomenon examinations based on various perspectives (Younas & Quennell, 2019). The theories provide general nursing ideas rather than a specific nursing intervention.
Middle Range Nursing Theories are more specific, focusing on and offering a more concrete connection between the nursing practice and grand nursing theories. These theories’ concepts are less abstract and thus more verifiable through testing (Younas & Quennell, 2019). Middle-range nursing theories try to explain, describe or predict certain clinical practice phenomena.
Practice nursing theories are situation-specific theories focusing on a particular patient and time. These theories provide the basis for nursing interventions and directly affect nursing practice compared to abstract theories (Younas & Quennell, 2019). They also provide suggestions on the expected outcome of nursing practice. Practice-nursing theories have their concepts interrelated to those of mid-range and grand nursing theories.
Application Grand Nursing in research
Yekefallah et al. (2017) apply Orlando’s nursing process theory to study the actions and activities of nurses in the management of anxiety. In the study, a quasi-experiment was conducted with a sample of 60 patients randomly assigned to two groups of 30 each and receiving care based on Orlando’s nursing process used. The first group received care based on Orlando’s nursing process theory, while the second one did not. The other group acted as a control group. The assessment was done on nurses’ actions during patients’ care by identifying behaviors associated with patient distress. In addition, Orlando’s nursing process principles were used to identify the dynamic interaction between the patients and nurses. In the research, an investigation was done on the anxiety differences for the patients receiving care based by evaluating nursing processes before the endoscopy examination, which was compared to a control group (Yekefallah et al., 2017). In addition, researchers used Orlando’s nursing process theory to provide information to the patients on the examination nature and sequence, endoscopy units and their structure, and the principles of operations.
Application of Middle-range Theory in Research
Brown et al. (2020) research theory use Merle Mishel’s uncertainty in illness theory to study the uncertainty existing in managing chronic illness. The theory is used descriptively to inform the researchers about the uncertainties prevalent when a patient cannot relate illness meaning based on the related events. The theory provided a framework for addressing the uncertainty in the diagnosis and treatment phases (Brown et al., 2020). Finally, the theory acted as the basis for identifying interventions geared towards the behavioral and psychological outcomes of the patients struggling with uncertain conditions.
Importance of Theory to the Advanced Nursing Role
Theory plays a critical role in advanced nursing practice. They are used to improve patients’ health outcomes by guiding the actions and decisions of nurses (Soyoun & Ju-Eu, 2021). Theory creates a framework for nurses to identify the patients’ needs through enhanced patient-nurse communication. Nurses using theories are guided to effectively assess a client’s health condition and design effective interventions for helping the patient. This is because theories shape the parameters of patient care delivery to align with the range of expected outcomes. The concepts and ideas of nursing theories are critical in nursing research and education. Theory helps nurses articulate evidence by justifying the methodologies behind the nursing practice (Soyoun & Ju-Eu, 2021). This is by equipping the nurses with skills and knowledge critical to providing high-quality healthcare.
How Research Informs and Advances the Nursing Discipline
Research plays a substantial role in informing and advancing the nurse discipline. It acts as a framework for generating evidence, thus advancing the profession. Research generates evidence on complicated healthcare systems and processes (Soyoun & Ju-Eu, 2021). This helps the nurses to determine the best nursing practice to improve patient care. Also, research is critical in generating new nursing information to correct any defects in the caring process while paving the way for new treatment methods. Finally, research is used to respond to changing healthcare environment, government regulation, policies, and patient population.
Conclusion
Theories have been critical role in shaping unique nursing concepts and practices. It helps nurses in articulating evidence while justifying the methodologies behind their practice. Theories have been used in research to guide and generate evidence (Hoeck & Delmar, 2018). They are also used to advance and inform the nursing profession by helping the nurses recognize and adapt the best practices that will improve the quality of care. Nurses are now adopting research to generate evidence-based practice that are driving clinical care and decisions. Recognizing and adopting theory in nursing will be critical in improving patients’ health outcomes.
