The purpose of this assignment is to demonstrate the skills of the professional nurse as an educator. You are to prepare a patient scenario based on the required assigned topic. You will choose an already developed and reliable mHealth app to use in the education of your patient.

Required Assignment Topic:
Select ONE of the Health Conditions or Health Behaviors for the focus of your assignment through the Healthy People 2030 link below: (Links to an external site.)
**Note: You will use the same scenario you develop and the same mHealth app you identify in future assignments (Weeks 4 and 6).** I will be choosing a designated writer.

Requirements and Guidelines:
1. Download the required Milestone 1 Template (Links to an external site.). **I have attached this**
2. Write a patient scenario using the Required Assignment Topic. Make sure to include a Healthy People 2030 Health Condition or Health Behavior in the scenario for a patient with a desire to maintain or achieve good health and prevent illness and who could benefit from an already developed and reliable mHealth app.
3. Choose a scholarly peer-reviewed journal article published within the last 5 years and related to the health condition or health behavior in your scenario.
4. Cite this in APA 7th edition format in this section of the assignment.
5. Describe in detail the plan for teaching this patient in the scenario.
6. Include the nurse’s assessment of learning needs and readiness to learn.
7. Identify a mHealth app that could benefit the patient, considering the teaching/learning needs you identified.
8. Describe the app including the mHealth app name, purpose, intended audience, mobile device(s) upon which it will operate, and where to download or obtain it. Add any other information you believe would be pertinent to this situation.
9. Include a working link if it is to be downloaded from a website in the area indicated on the template.

Case Scenario

Mr. D is a 52-year-old African American. He was diagnosed with Type 2 diabetes at the age of 46. Since then, the patient has been on insulin. Mr. D confirms a family history of diabetes from the maternal side. In the previous visit, the patient was diagnosed with hypertension. Besides, the patient has other health issues, which include being overweight. He confirms to lead a sedentary lifestyle. Mr. D works as a truck driver and operates for long hours. His occupation hinders him from engaging in regular physical activities.

Teaching Plan

The patient needs health education to enhance his knowledge about his well-being. Also, the health education program will strengthen his self-care management skills, allowing him to manage his health conditions effectively using various measures. Therefore, the teaching program will focus on particular areas to help achieve the above targets. First, the patient will learn about the health problem. Here, the patient will be taught the signs and symptoms of T2D, its cause or risk factors, the health complications associated with the disease, and its treatment (Xu et al, 2018). Secondly, the patient education program will focus on utilizing technology in diabetes management. Here, the patient will learn about various mHealth apps applied to manage type 2 diabetes and their features. The patient will learn how they work, their key features, where he can find them, their cost, and the safety measures he can utilize to protect his electronic information stored and transmitted using the application.

Nurse Assessment of Learning Needs and Readiness to Learn

The patient’s readiness to learn will be determined in various ways. First, the patients will be asked if he has enough free time to attend the training program. Secondly, the patient will be asked if he can communicate the language that will be applied during the training.

mHealth app and its Benefits

The Glucose Buddy app would benefit the patient. This mHealth app has various features that will enable the patient to manage his health effectively. First, this app will allow the patient to track blood glucose levels (Glucose Buddy, n.d). Being a T2D patient, he experiences unstable blood glucose levels. Therefore, this mHealth app has various analytical features that will enable the patient to record and analyze his blood glucose levels, analyze their trend, and formulate informed decisions to maintain their health. Second, MR. D is under insulin medication. This might experience different challenges, such as a lack of compliance with his prescribed diabetes regimen. The Glucose buddy will allow the patient to create reminders that will alert him to take his medications as defined by the physician (Glucose Buddy, n.d). Thirdly, the patient faces a challenge in managing his weight and food intake. The mHealth offers a remedy to this need. It will help the patient to track his physical activity and food intake. Lastly, mHealth can enhance communication between the patient and his physician. The mHealth app comprises a chart display that can allow the patient to identify patterns and gain insight into his blood glucose trends. Besides, the patient can create printable reports which he can share with a healthcare professional (Glucose Buddy, n.d).

This is a mHealth app whose target audience comprises individuals with diabetes. Its purpose is to enhance the management of diabetes by tracking blood glucose levels, insulin medication, carb intake, and A1C results. This app can operate on both android and apple mobile devices.
