Know Stroke is one of the national health campaigns. It was developed by the National Institute of Neurological Disorders and Stroke (NINDS) in 2001 (National Heart, Lung and Blood Institute, n.d). The goals of the campaign are to help educate the public concerning stroke. It majorly informs the public about the symptoms of a stroke. Secondly, it focuses on informing the public why it is necessary to receive medical care quickly. Therefore, it encourages the community to contact 911 in case of stroke. Also, it aimed at influencing the healthcare community to establish and improve protocols to treat the disease. The campaign targets individuals at high risk for stroke, majorly Hispanics, African Americans, adults aged 50 years and older, and Spanish-speaking communities. Also, it targets their family members and caregivers, and healthcare professionals, since the individuals experiencing the disease are unable to act on their own to contact 911 and search for health care (National Heart, Lung and Blood Institute, n.d). Besides, the campaign involves various activities, including outreach to consumers and medical staff through mass media,  community education, affiliations, and grassroot outreach.

Know Stroke has been effective in addressing the health issue. One of its achievements includes the establishment of a vital partnership with the CDC and Stroke Prevention which developed Know Stroke in the Community (KSIC) in 2004.KSIC employed well-trained stroke champions in every society (NIH, n.d). Secondly, the campaign targets the family members of those at high risk for stroke to help people suffering a stroke get to the health facility promptly and receive time-sensitive treatments (NIH, n.d). However, the program can use various information technologies, such as the internet to reach more people, to enhance its effort of addressing the health issue (Stark et al.,2022).

The Heart Truth is a national public health campaign designed to raise awareness about heart disease. One of the goals of this campaign includes increasing awareness and expanding to reach women of color aged 40-60-years-old. Also, it is committed to informing the general public about heart health, especially those individuals impacted most by the disease. The target audience includes younger women, African Americans, and other minorities. The campaign has been effective in fighting against heart disease. Evidence shows that the campaign has helped increase women’s heart disease awareness has increased significantly since the launch. Secondly, this program has developed the Healthy Hearts Network, which has helped reach local communities and work together with health and community health professionals on heart health education programs. However, heart disease remains a leading cause of death among women of color. More effort need to be made in informing individuals at risk of heart disease on how to access care (Alkhouli, et al., 2019).

One of the key elements of a successful community-based action plan is evidence-based. Evidence-based practices are instrumental to having a strong community-based action plan and also enhance their success (Rollins et al., 2020). Thus, the practices included in the plan should be based on the best available evidence from research and individual clinical expertise. This can help avoid wasting time and resources utilized in implementing a less effective program. The second key element is community engagement. An effective plan should engage different stakeholders within the community. This helps increase connection to the community, increase knowledge about community issues, and focus on addressing the problems (Rollins et al., 2020). Lastly, the evaluation plan is also important. This helps in determining the effectiveness of the program (Rollins et al., 2020).
