Choose a few health issues in your community that can be used to plan community health interventions. Briefly identify urgent health issues and non-urgent health issues. What makes health issues urgent and a priority? Which needs are not a priority and why? Who determines this or should determine this (identify internal and external stakeholders)? How would you document and present this to your community?

When reviewing implementation strategies, it is important to remember to make effective use of goals and objectives. These must be measurable and realistic. Using the SMART tool to set objectives has been helpful in achieving success for many leaders. Define what SMART stands for and describe each step. How will it help define your specific goals and objectives? How will it help you tailor strategies to a specific community culture? How can tailoring lead to more successful intervention? Finally, how will having clear objectives help in designing a community health assessment?

Discussion 3

When a draft community health assessment is designed, the next phase is implementing it. Discuss what an action plan may look like from gathering data to putting it into action. Write in steps what you would do. What priorities did you identify and why? How did you gather the data? Is your plan supported with research? Is the plan flexible or set in stone? Is the timeframe in meeting the goals realistic? What community members are involved and what are their roles? How would you put your plan into action?
