Nurses make up the most significant segment of the US healthcare workforce. Although the nursing role has been traditionally assistive to the physician, the trend is increasingly changing. The introduction of advanced nurse training programs results in graduate nurses capable of independently performing advanced clinical roles previously reserved for physicians. This is proving promising in meeting the current physician shortage across the US (Schober, 2018). The practice scope of these advanced trained nurses varies by state. While others have implemented laws allowing advanced nurses full practice autonomy, others are still reluctant, thus requiring these Advanced trained nurses to collaborate with a physician. However, learning from the states that have allowed full ANP practice autonomy, more and more states continue to embrace the policies. Advanced Nursing Practice (ANP) and the Advanced Practice Nurse (APN)
ANP and APN are two different but closely related concepts. ANP is an umbrella name for advanced nursing practice that entails incorporating full in-depth nursing knowledge, skill, and experience in meeting clients’ health needs. The ANP role is guided by the principles of a high degree of knowledge, skill, and expertise. On the other hand, APN refers to a nurse with advanced training, primarily post-graduate education and certification in a master’s degree or a doctorate in nursing. Nurses are practicing at this level work in either a specialist or generalist capacity. The role of APNs includes diagnosing and treating illnesses, providing advice on public health issues, providing health education to the public, managing chronic disease, and engaging in medical research (Schober, 2018). They also prescribe medications and take part in nursing administration.
APNs are equipped to offer patient care in diverse areas of specialization, including pediatric nurse practitioner, family nurse practitioner, adult nurse practitioner, certified nurse-midwife, certified registered nurse anesthetist, and gerontologic nurse practitioner. ANPs are one of the main types of APNs. Nurse practitioners are APNs who work with patients in the outpatient setting. They work closely with diverse patient populations, including adults, elderly, pdiatrics, men and women to provide preventive care and regular checkups. . they also work as primary care health providers for many patients, where their roles entail routine health maintenance and treating occasional simple health problems. The NP may have several diagnostic medical tests as part of the preventative care. Certified Nurse-Midwives provide prenatal and postnatal care to women and babies. They may work independently or with physicians, depending on the state laws. Nurse anesthetists provide patient care in the surgical operating room (Schober, 2018). They administer anesthetics during surgery and monitor vital signs during and postoperative. Clinical nurse specialists often work as part of a multidisciplinary medical care team. Their roles include making diagnostic and medical care decisions. They may work with the general population or with a special population.
My Desired NP Role
I aspire to study the advanced nursing practice role as a family nurse practitioner (FNP). The FNP scope of practice entails providing primary and specialty care of patients of all ages, either independently or collaborating with physicians. The role of FNPs in general care setting include preventive care, diagnosis and treatment and disease management, and health prevention. The rationale for selecting this special is that FNPs can work across different specialties in various healthcare environments and care for patients of all ages. Therefore, pursuing an FNP program will increase my chances of employment and positively impact multiple patient populations. My excellent communication skills and attention to detail will be essential in providing safe and quality care to diverse patient populations. These skills are crucial for inaccurate diagnosis and promote better patient experience and satisfaction.
My philosophy of nursing has also evolved throughout my nursing career. Currently, my nursing philosophy is characterized by holistic care. I believe proper nursing entails understanding patient needs at all levels, including physical, emotional, and psychological. Therefore, it involves caring for more than the current patient’s clinical presentation. I also believe that nursing care should extend to the patient’s family and community, as they for the patient’s environment, which shapes their health. Therefore, in my role as a nurse, I focus on providing holistic care rooted in evidence-based practice, solving various issues to get to the root cause of their illness, and being their advocate.
I believe that through an understanding of a patient’s holistic needs, a nurse can adequately advocate for patients’ needs. I also engage in interdisciplinary collaboration in providing holistic patient care to ensure safety and quality. My nursing philosophy is based on the nursing theory I relate most with: Florence nightingales’ environmental theory. In theory, the theorist emphasizes the holistic aspect of nursing. According to Nightingale, various factors within the patient environment influence their health, as well as the care outcomes (Riegel et al., 2021). Understanding the health factors within the patient environment helps address the health problem from the root. Therefore, I utilize both clinical knowledge and nursing models in patient care.
Regulatory and Legal Requirements
Since I will be practicing in Florida, the following legal and regulatory requirements bind my scope of practice. To begin, I need to have a current, unrestricted license. Second, has not been disciplined by any state or jurisdiction in the past five years prior to registration. Third, prior to registration, must have completed at least three graduate-level semester hours in pharmacology, 3,000 practice hours, and both of which must have been accomplished within the previous five years. As an FNP in Florida, I am allowed to operate independently provided I have a documented agreement with a physician, according to the Florida Department of Health and the Florida Board of Nursing.
Required Competencies and Certification
As an FNP, several competencies and certifications are required for practice. Some of the notable competencies include technology and information literacy, leadership, independent practice, practice inquiry, scientific foundation, ethics, quality, and population role competencies (Tennyson & Smallheer, 2022). The only required certification in my role is the FNP certification from the American Nurse Credentialing Center.
Requirement To Obtain Certification
Family Nurse Practitioners can get their FNP certification by passing a test that measures their clinical expertise and knowledge. Accreditation of an FNP requires a valid Registered Nurse license (RN) with at least 500 clinical hours and an approved master’s, post-graduate, or doctorate degree from a recognized FNP institution accredited by Commission on Collegiate Nursing Education.
Transformational leadership refers to a style of leadership that affects both social systems as well as individuals. It enhances promising and quality transformation of employees in its optimal form, aiming at transforming employees into leaders (Collins et al. 2019). Family Nurse Practitioners are not expected to be the smartest people in the field. Their curiosity, not their intelligence, motivates them to come up with novel answers to issues that have been known for a long time. They rely on professionals on their team when implementing plans to help them realize their vision. Transformational leaders utilize a strong vision to view the big picture as well as spot the minor roadblocks to efficient performance (Collins et al. 2019). Transformational leaders have an instinctive knowledge on how to take their roles so that to be competitive. Because they care, most of them are considered important. Some of the transformational leadership attributes that I possess include visionary, charismatic, simplicity, team-first attitude, and curiosity.
The best leadership style as an APN in primary care is situational leadership. As an APN in primary care, I am certified to attend to patients without a doctor and make critical medical decisions. Therefore, situational leadership style will help me appreciate the fact that different will require different leadership styles. For instance, transformational leadership will help model ethical behavior in my own practice and urge others to do the same, and establish an ethical workplace with defined priorities, standards, and values.
Due to more patients living with chronic diseases and an aging population who require timely access to high-quality care, the APN role in primary care is currently facing an economical and quality crisis. Additionally, professional shortages jeopardize the system’s capacity to achieve demand for care. Restriction on APRN practice, inadequate recognition of the profession, insufficient knowledge on the role of an APRN, opposition by physicians to APRN practicing independently without physician oversight or supervision, and poor APRN–administration relations are all current policies affecting the APRN practice environment. These current trends need to change so that APN’ role in primary care is maximized.
How APN can Lead Changes
Advanced practice registered nurses have crucial responsibilities in leading the above-mentioned change. First, they should serve as advocates demanding professional recognition. They can achieve this by forming an international movement to air their grievances. In terms of professionalism shortages, they can lead the change by advocating for more trainees as well as advocating against those policies restricting the advanced nurse practices. In addition, they should demand more financial budget allocated to primary care. This budget should be used to hire and recruit more advanced practice nurses into the role. In order to increase and boost their relationships with other healthcare providers, they should lead by establishing rapport as well as practicing good behavioral conduct all the time.
Family nurse practitioners play a vital role in providing primary care. The FNP scope of practice entails providing primary and specialty care to patients of all ages, either independently or in collaboration with physicians. Since the practice scope of advanced trained nurses varies by state, it is essential to familiarize with each state’s regulatory and legal requirements. By doing this, one will be able to carry their roles and practices smoothly. For instance, it is a requirement to have a certification of accreditation in order to practice as an FNP in Florida. The main difference between Advanced Nursing Practice and the Advanced Practice Nurse is that advanced nursing practices lead to one becoming an advanced practice nurse.
